How do I apply for BridgeWell? Complete the form located on the left-hand side of this page. We will send you the application forms by mail, fax, or email, whichever you prefer. Complete the application packet and return it to us, we will contact you once your application has been evaluated and assist you in coordinating the medical care you need.
How long does the application process take? It takes us 24 hours to underwrite and evaluate your account once your application packet is completed and submitted to us. It is important to include all of the information we request to speed up the process. Once approved, you can be scheduled immediately for the healthcare service you need.
Can I call BridgeWell directly to apply for BridgeWell or do I have to go through my healthcare provider? Yes you may contact BridgeWell directly. If you have a specific doctor or facility you would like to use, make us aware of that when you contact BridgeWell. If you do not have a specific doctor or facility you would like to utilize, BridgeWell can provide you with a list of healthcare providers that will suite your needs.
Are there any out of pocket costs, additional fees, or interest to the patient for BridgeWell? There are no out of pocket costs, additional fees, or interest. Once approved for the program, there is no cost to using BridgeWell.
Who pays for the healthcare treatment the patient receives through BridgeWell? The at-fault insurance carrier should pay for the amount of the lien as a part of your personal injury settlement.
